Cat Adoption Application

A submitted application does not guarantee adoption of a dog or cat from Our Farm Sanctuary (OFS). Please make sure all fields are completed, including veterinary reference if you already have pets, failure to fill out the application in its entirety could result in a delay of processing your application. You will be contacted by an OFS representative to discuss adoption only if initial review of your application is satisfactory. (Please note we are 100% volunteer, applications are reviewed and applicants called as quickly as possible, we appreciate your patience with our entirely volunteer staff.)

The animal an applicant would like to adopt from OFS may not be the one best suited to meet the applicant's needs.

OFS reserves the right to offer an animal they feel is better suited to match the application. Animals are placed in the best home, not the first home that requests them.

By submitting this application, I certify that the information provided is truthful and correct to the best of my knowledge.

By submitting the application, I affirm that

(1) I have read the Our Farm Sanctuary (OFS) adoption policies,
(2) I agree to be bound by those policies,
(3) I understand that the decision to accept this or any application, or to place any particular dog or cat with any particular applicant is at the exclusive discretion of OFS,
(4) OFS may speak with the applicant’s vet, and
(5) all dogs and cats are now and remain the property of OFS and must be returned to OFS upon demand. Demand will only be made for cause—i.e., violation of contract—as a protection of the animal.
